An organisational free-for-all is in full swing as start-up GP consortia establish new commissioning arrangements.Some clinical commissioning groups (CCGs) are forming American-style commissioning senates. Some primary care trust (PCT) clusters are delegating budgets to a single consortium, while certain private GP providers say they are working exclusively with 'like-minded practices' - whether this latter move excludes the public sector is unclear.Although the government promised nurse involvement in CCGs, recent authorisation criteria make no mention of it (see resource file).
