Consensus on evidence-based implementation of early supported discharge.

The traditional organization of stroke care has been challenged by evidence-based medicine over the past few decades. The benefits of stroke units were questioned,1 but this model became part of the standard of care for acute stroke following clinical trials that clearly showed its benefits.2 The organization of stroke rehabilitation has also faced the need for evidence-based support, and in the mid-1980s a model of early supported discharge (ESD) was suggested.3 This model involved accelerated discharge of stroke patients with mild to moderate deficits, who would then be rehabilitated at home as opposed to receiving institutional rehabilitation.
