Throughout his 30 year career in the minerals sector, Ashby explained, he had never had a fatality under his watch. This all changed, however, on July 29 in 2008 when a worker died on site - an incident which sparked a horror run that involved four more Pilbara deaths over the following 10 months. During his appearance at the popular Kalgoorlie-Boulder show, Ashby said these unfortunate developments had demonstrated to the company that its usual safety indicators were not effective when it came to controlling fatality exposure.
