Phenotypic switching in diabetic gastroparesis: mechanism directs therapy.

Recent advances in neurogastro-enterology have significantly increased our understanding of the pathophysiology of the symptoms of diabetic gastroparesis. Many of these findings may be translated to new approaches in the treatment of this difficult disorder. It is now quite clear that not all of the symptoms reported by patients with diabetic gastroparesis are due to delayed gastric emptying.3 Furthermore, patient symptoms may switch over time, suggesting that different pathophysiologic mechanisms may predominate at different times. This phenotypic switching over time has important implications in our treatment strategies for patients with diabetic gastroparesis.
