There is a well-recognized association between erectile dys function (ED) in men and the presence of type 2 diabetes, along with other cardiovascular risk factors. This communication represents an ancillary study based on sexual function in a subset of participants from the US National Institutes of Health (NIH)-funded Look AHEAD (Action for Help in Diabetes) trial. The Look AHEAD trial included >5000 men and women with type 2 diabetes, aged 45-74 yr, who were overweight (body mass index >25). Participants were then randomized to inten sive lifestyle modification or to the usual support and education arm to evaluate the effect of intervention on cardiovascular morbidity and mortality. A subset of 373 of the men partici pated in the current study in which baseline data were obtained to evaluate sexual function (International Index of Erectile Function questionnaire), self-reported phosphodiesterase type-5 inhibitor use, adiposity, cardiometabolic parameters, and exercise fitness. Approximately 75% of these men with type 2 diabetes had ED: 50% of men had mild or moderate ED, while nearly 25% suffered severe ED. The presence of metabolic syndrome (age-adjusted odds ratio [OR]: 3.05) and history of hypertension (age-adjusted OR: 2.41) were found to be highly associated with ED. Exercise fitness level was found to be protective of erectile function (age-adjusted OR: 0.76).
