Do We Need to Visually Examine the Inside Surfaces of Piping Welds?

When called upon to visually examine weld's in a piping system (Fig. 1), what surfaces of the welds must be examined? Is it just the external, out-side surfaces of the welds and heat-affected zones (HAZs), or also the internal, inside surfaces? Is a mirror or borescope required to visually examine? Several codes and standards are evaluated here to determine the answer.
