A 58-year-old man presented with cerebral infarction. Echocardiography showed a 10-mm floating intra-aortic mass in the ascending aorta, which disappeared after 10 days of antiplatelet therapy. Two months later, a 6-mm string-like mass was detected at the tip of the noncoronary cusp and surgical removal of the string-like mass was scheduled. At surgery, the intra-aortic mass was unexpectedly discovered as a flat thrombus adhering to the normal ascending aorta (Figure 2). Histology confirmed the string-like mass as a papillary fibroelastoma, and the intra-aortic mass as an organized thrombus. The thrombus and the fibroelastoma were 3 cm apart and thought to be unrelated.
