Isoniazid prevented active tuberculosis in patients with HIV treated with antiretroviral therapy: Commentary

Worldwide, TB is the leading opportunistic infection and cause of mortality in people with HIV infection. A 2010 Cochrane review found that any LTBI regimen reduced TB disease by 32% in patients with HIV and by 62% in patients with HIV and positive results on a tuberculin skin test (TST) (1). The RCT by Rangaka and colleagues adds to the evidence that treatment of LTBI is beneficial in people with HIV. It does not, however, resolve the issue of whether LTBI treatment should be given to all patients with HIV infection or only to patients with positive results on a TST.
