Empagliflozin-Associated Fournier Gangrene

A 45-year-old female known case of type 2 diabetes mellitus was on tab metformin 1 gm twice daily and glipizide 10 mg twice daily. She had poor gly-cemic control, and empagliflozin was added to her medication regimen to achieve glycemic control. After a period of 1 month, she presented to the emergency department with redness and burning sensation over the genital area along with groin pain for the past 1 week, which was gradually increasing in severity. On examination, she was afebrile and hemo-dynamically stable. Genital examinations showed red indurated skin over the genital area and perineum along with bilateral inguinal lymphadenopathy (Figures 1 and 2). Investigations revealed elevated inflammatory markers and HbA1C of 12%.
