Which Time Is It? A Suggestion for Unambiguous Nomenclature in Transplantation

We applaud the efforts of Bernat et al. (1) in trying to define and clarify the different phases of ischaemia during organ donation. Accurate recording of data relating to transplantation is essential for audit and research, and to provide a common language of transplantation yet 50 years after the first successful renal transplant, there is still confusion about the nomenclature of time points in transplantation. We propose that 'ischaemic' terminology needs further clarification and internationally approved definitions to standardize nomenclature and allow accurate comparisons of results. The suggested use of withdrawal phase {Phase I) and the acirculatory phase (Phase I!) is sensible but this classification needs to be extended to clarify the remainder of donor organ ischaemia.
