Does the way we organise anaesthesia departments matter? What should be the measures of outcome?

Does the way we organise matter? Do ways in which we structure processes, procedures, and organisations affect anaesthesia and intensive care accomplishments? Our successes? Our failures? My guess is that most anaesthesiologists think that this is the case. But they would probably not agree on what might be achieved by implementing a certain organisational style or by changing an organisation.
