Central pontine myelinolysis (CPM) is an uncommon condition usually seen in patients -with underlying conditions such as chronic alcoholism, malignancy, malnutrition, and hyponatremia [1]. We recently encountered a patient with AIDS and neurological abnormalities, whose evaluation was consistent with CPM.A 38-year-old female patient with AIDS (most recent absolute CD4 cell count of 14cells/|Jil) presented with a 4-week history of dysarthria, dysphagia, increasing generalized weakness, blurred vision, confusion, and difficulties with balance and walking. She was noncompliant with her prescribed medications (lopinavir/ritonavir 400mg/100mg twice daily, emtricitabine 200 mg daily, tenofovir 300 mg daily, atovaquone 1500mg daily .and azithromycin 1200 mg weekly)
