Letter to the Editor: Management of Concurrent Pregnancy and Acute Lymphoblastic Leukemia or Lymphoblastic Lymphoma.Comment on the Article by Zaidi et al.

IN a comprehensive review, Zaidi and her colleagues offer well-considered recommendations for the management of acute lymphoblastic leukemia (ALL) and lymphoblastic lymphoma (LBL) during pregnancy.1 The opening sentence in the abstract is somewhat off target, however, since the young adults are denned as <22 years of age in reference to the "usual age range" of lymphoblastic malignancy. ALL and LBL occur in "young adults" up to as high an age as "young" may apply (Fig. 1). Moreover, the problem of treating ALL and LBL during pregnancy is more relevant for young adults up to a much higher age than 22 since (1) there are just as many women diagnosed with these diseases between 22 and 35 years of age than those diagnosed between 15 and 22 years of age (Fig. 1); (2) there are more pregnancies in the latter age group than the former3; and (3) chances are that the older women will receive chemotherapy with an adult regimen that is more likely to have adverse effects on the embryo, fetus, or mother,4 the latter evident in case 1 presented by the authors. The relevance of the review and excellent recommendations are not limited, as stated in the title, to teenagers.
