摘要

Due to the silos organization in hospitals, both the clinical wards and the pharmacy face hidden costs. A proactive approach can be initiated by the pharmacist to favor cooperation and implement concrete actions to improve patient medication use process. A structured and methodic approach is required such as the socioeconomic approach to management (SEAM) including listening to the clinical wards and pharmacy staff to identify the dysfonctionnements due the lact of cooperation and thus figure out the related hidden costs. This paper illustrates some achievements obtained after some of our research action interventions carried out in Hospital Pharmacies. It shows, for example, how the technician pharmacists activities can save nursing time, improve global patient care, and finally increase satisfaction or well being of both clinical ward and pharmacy staff. It only requires for the chief pharmacist to lead the change by selecting two clinical wards and convincing them to commit in the change process.
