摘要

Heart failure (HF) is a chronic, progressive and terminal illness with poor median survival and high mortality rates. The unpredictable and life-limiting nature of HF suggests that this population would benefit from advance care planning (ACP). Advance care planning is the process whereby individuals reflect on and articulate personal values and goals and identify a substitute decision maker to inform goals of care discussions, medical treatment and end-of-life care. Research has demonstrated that HF patients have limited awareness and involvement in ACP, and many studies seek to improve ACP participation by utilizing Registered Nurses and/or Advanced Practice Nurses as facilitators of the ACP process.
