Surgical strategy for treatment of bilateral temporomandibular joint ankylosis

Temporomandibular Joint (TMJ) ankylosis is a pathogenic disorder where the mandible is fused to the fossa by bone fusion or fibrous adhesion1. The main aetiologies are trauma, infection and systemic diseases with local repercussions in TMJ, such as severe limitation of mouth opening, pain, masticatory limitations, difficulty in oral hygiene, facial asymmetries, malocclusion and even upper airway compromise2"4. There are many treatment protocols described. This report shows a successful treatment strategy for treatment of a bilateral TMJ ankylosis in a 38-year-old male patient complaining of pain in TMJ bilaterally, feeding and speech difficulty treated at the "Hospital das Clinicas de Teresopolis." Clinically, he presented a mouth opening of 24 mm and complete edentulism (Fig. 1A). He reported motorcycle accident 5 years ago. The computed tomography revealed bilateral TMJ ankylosis and sequelae of a mandibular symphyseal fracture (Fig. IB).
