After birth, numerous physiologic changes must rapidly transpire for the fetus to successfully make the transition to a neo-nate. Despite the complexity of this process, on average, only 6% of newborns born in the United States require life support in the delivery room. This percentage rises quickly, however, among newborns who weigh less than 1500 grams. Given the relative rarity of the need for full resuscitation, it is essential that all delivery room personnel frequently review the neonatal adaptations to extrauterine life, make provisions for resuscitation, understand the predictors of the need for resuscitation, and know how to respond appropriately.
