The Importance of Strict Perioperative Glycemic Control: Lessons From the Portland Diabetic Project

Although only 6% of this country's population has diabetes, they make up a disproportionate amount of the coronary bypass population (29%).This is because diabetic patients are at increased risk (54%) of acquiring treatable coronary artery disease as compared with the nondiabetic population (5%).In addition to this, patients with diabetes have been reported to have higher rates of postoperative infection (3.5% vs 0.5%) and death (4.5% vs 2.7%) than the nondiabetic cardiac surgery population. Hospital lengths of stay average 1.7 days longer; in addition, long-term survival after coronary artery bypass grafting (CABG) is diminished. These discrepancies in cardiac surgery outcomes have been termed the unfair disadvantage for patients with diabetes.
