Distal migration of a peritoneal catheter to the lumbar and sacral region is a very rare occurrence in the literature. Only one case is described showing a lateral lumbar migration and output [6]. We present a peritoneal catheter migration of a ventriculoperitoneal (VP) shunt (VPS) to the center of the sacral region, including radiological documentation of catheter extrusion through the sacral bone. A 5-year-old boy presented to our institution complaining of lumbar pain and with the appearance of swelling and hyperemia in the middle part of the lumbar region for the previous 8 days. His mother informed us that she had seen part of the catheter in that area; however, we did not observe this occurrence. He returned 2 weeks later with the distal part of the peritoneal catheter extruding from the swelling (Fig. 1).
