A 4-year-old Chinese boy presented to a paediatric accident fit emergency department with a 9-day history of fever and diarrhoea (without blood and mucus). His parents had been treating him with water and paracetamol. His younger sister recently had diarrhoea and vomiting. His observations were all normal and examination was unremarkable. He was discharged with reassurance he was not dehydrated following a negative urine dipstick.He returned 2 days later because his parents were worried that his fever was persisting despite paracetamol. On re-examination his liver was now palpable. Clinically he was not dehydrated, handled well and had no suspicious rashes.
