Rise in number of patient safety incidents reflects improved reporting rate, says NPSA

NHS staff are reporting more patient safety incidents, the National Patient Safety Agency has concluded. The view emerged as the agency published Organisation Patient Safety Incident Reports for each NHS trust, or local health board, in England and Wales.
