Holistic approach to health care brings success

A runner-up prize was awarded to a supplementary prescribing pharmacist who has successfully integrated into the primary care team to provide a holistic approach to chronic disease management.The new general medical services contract and the introduction of pharmacist supplementary prescribing in 2004 was the impetus for practitioners at Norton Medical Centre to develop new methods to deliver patient care.One of these is an annual care review (ACR),with patients attending the surgery each year for a holistic review of their health.This is carried out by a health care assistant who collects the data for each patient,according to a template,and identifies problems such as out-of-range blood test results and medicines compliance issues.
