Pruritic papular eruption and blisters in a 5-year-old girl

A 5-year-old white girl presented with a pruritic papular eruption that had begun 3 weeks previously on her legs, and gradually progressed to the arms, trunk, palms and soles (Fig. 1). The child had a history of atopic dermatitis, and of sensitization to peach, plum and egg. One week later, the patient returned to us, presenting with tense bullae that had appeared on previously uninvolved areas on her toes, and on the pre-existing lichenoid papules on the left leg (Fig. 2). On physical examination, multiple flat-topped papules were seen on the patient's trunk, arms, legs, palms and soles, in addition to tense blisters at the locations mentioned above. Nikolsky sign was negative.
