Comment on 'The broken pen penalty' (Lakshmanadoss et al.).

The recent publication by Lakshmanadoss et al. reported a case of a malfunctioning insulin pen, which they associated with poor glycemic control, in a woman with type 2 diabetes. In this case, pen malfunction may have contributed to poor glycemic control, but this case should be viewed as the exception of insulin pen use rather than the rule. Published data on accuracy, safety, and efficacy of insulin pens strongly support their use. The current case report should be viewed with caution and not used to influence decisions on pen versus vial and syringe for patients who will benefit from insulin therapy.
