Well-designed and well-conducted randomized clinical trials provide the best evidence to guide clinical practice and form the backbone of clinical guidelines. Polio vaccination, aspirin for acute myocardial infarction, tacrolimus for transplantation, and many other now-routine treatments stem from landmark trials that changed practice and improved outcome.
