I read with interest the article by Kroma et al (1) regarding a novel transjugular intrahepatic portosystemic shunt (TIPS) flow reduction technique with the iCAST balloon-expandable stent (Atrium Medical, Hudson, New Hampshire). A variety of reducing techniques for challenging TIPS-related hepatic encephalopathy have been reported. Unfortunately, these are mostly limited to case reports and small retrospective series, thus making comparisons between techniques and outcomes difficult to achieve.
