Endografting the true lumen and false lumen of an enlarging infrarenal aortic aneurysm.

We present a case of a unique endovascular treatment of a chronic dissecting abdominal aortic aneurysm in a patient with Marfan syndrome who refused open surgical repair. In this patient, the left iliac artery was supplied by the false lumen of the aneurysm, which was rapidly enlarging. The false lumen was compressing the true lumen that was supplying the right iliac artery. We needed to prevent compression of the true lumen and preclude continued enlargement of the false lumen. The proposed option for treatment was to implant stent grafts into the false lumen and the true lumen. Findings at 12-month follow-up included patent stent grafts, no endoleak, and decrease in the size of the aneurysm.
