摘要

The Third Bieleschweig Workshop on System Engineering took place February 12-13,2004,at the Center for Interdisciplinary Studies (ZiF) in Bielefeld,Germany.Using the Why-Because Analysis method,root cause analyses were presented of the 1994 Operation Provide Comfort downing of two U.S.Army helicopters in Northern Iraq by two U.S.Air Force F-15s,and also of the mid-air collision of two commercial aircraft in 2002 over Lake Constance,Germany.
