Re: Systematic pelvic lymphadenectomy vs no lymphadenectomy in early-stage endometrial carcinoma: randomized clinical trial.

In the report by Benedetti Panici et al. (1), results of a randomized trial on lymph-adenectomy in patients with endometrial cancer are presented. The authors appropriately acknowledge the main limitations of their study (ie, the lack of systematic para-aortic lymphadenectomy and strict criteria for adjuvant therapy), but we believe that comments related to the following three topics are necessary to avoid misinterpretation of the study: inclusion of low-risk patients, incomplete surgical procedure, and lack of standardization of postoperative treatment.
