Intravascular Lipoma of the Right Innominate Vein in a Trauma Patient

A 58-year-old man whose horse fell on him after he fell from the horse, complained only of left-sided chest pain. CT scans revealed multiple left rib fractures, bibasilar atel-ectasis, left L3 transverse process fracture, right acetabular fracture, and a filling defect with fatty tissue attenuation within the right innominate vein (A, arrow). He was admitted to the trauma intensive care unit for pain control and aggressive pulmonary toilet. On hospital day 3 he had operative reduction with internal fixation of the right acetabular fracture. The patient did not complain of right arm pain or swelling before the traumatic event, and CT scan of the chest showed adequate collateralization of the right . innominate intraluminal mass
