Claiming that 98,000 Americans died annually from medical errors, the 1999 Institute of Medicine (IOM) [1] report To Err Is Human: Building a Safer Health System rocked the country. The magnitude of injury was almost unbelievable: more fatalities than AIDS, breast cancer, or automobile accidents. The mortality rate was equal to that of a commercial airliner crashing every day for a year.
