The extended fleur-de-lis latissimus dorsi flap: A novel flap and approach for cover age of lower back defects

First described in the early 1900's,1 the latissimus dorsi has been a workhorse myocutaneous flap utilized in breast reconstruction. However, a major limitation of this flap is the limited bulk that it supplies often requiring simultaneous prosthetic placement. Modifications to the latissimus flap, aimed primarily at increasing volume, have been previously described and have evolved into the extended latissimus dorsi flap.2 The extended flap accomplishes increased volume by including the parascapular, scapular, and lumbar fat with reliable results.3 Amongst the many modifications that ultimately led to the extended latissimus dorsi flap is the fleur-de-lis skin paddle design which was first proposed in 1991.4 Utilizing a novel extended fleur-de-lis latissimus dorsi flap, we present an alternative method for coverage of extensive lower back defects.
