Could intravenous cyclosporin A be an effective therapeutic strategy in pulmonary hemorrhage associated with Henoch-Schonlein purpura? Comment on: Pulmonary hemorrhage associated with Henoch-Schonlein purpura in a child (Clin Rheumatol 27(6):803-805).

We read with interest the article by Matsubayashi et al. [1]. They firstly used intravenous cyclosporin A (CyA) in a 6-year-old boy with pulmonary hemorrhage associated with Henoch-Schonlein purpura (HSP) with a favorable result [1]. However, they could not explain possible mechanisms. We speculate that (1) suppression of immunoglobulin A (IgA) production by CyA [2, 3], (2) the endothelium-dependent vasoconstriction effect of CyA [4], and (3) the inhibition of various inflammatory cytokines by CyA [5, 6] might control the severe pulmonary vasculitis in this case.
