We implant the CTR when the capsule retractors are in place and after the cortex has been carefully removed. Due to the aforementioned arranged placement of the capsule retractors, the subsequent implantation of the CTR occurs without extra pressure on the zonule. The following technical approach was performed in all eyes in this study when zonular weakness was identified intraopera-tively; eg, increased lens movement prior to the cap-sulorhexis and/or capsular wrinkles during capsulorhexis. In some cases, iris retractors were needed to complete the capsulorhexis. Regarding the question of when the OVD located posterior to the implanted IOL was removed: In all cases, prior to IOL implantation, we injected the OVD into the anterior chamber; after performing IOL implantation in the sulcus and IOL optic capture through the anterior capsulorhexis, we removed the remaining OVD.
