A multifactorial hypothesis to explain sudden cardiac death (SCD) proposed that malignant ventricular arrhythmias arise from three concurrent conditions: a structural-functional substrate, an arrhythmic mechanism, and a transient initiating event.1 A well-known example of this theory is the onset of fatal arrhythmias in patients with congestive heart failure (CHF). In CHF a proposed structural substrate is postis-chemic dilated cardiomyopathy; the arrhythmic mechanism, a reentry or a triggered beat; and, ultimately, the transient initiating event, a psychophysical stress that acts by increasing sympathetic activity and reducing vagal modulation#
