Contained left ventricular rupture with left atrial dissection after mitral valve repair.

LEFT VENTRICULAR (LV) RUPTURE following mitral valve (MV) replacement is a rare but potentially fatal complication, with a mortality rate of more than 50% in current reported series.1"4 Several authors have described left atrial (LA) dissection following MV replacement; however, the literature search for the current article could not identify a single case of LV rupture associated with an LA dissection following MV repair.57 In this report, a contained LV rupture after MV repair is described. The differential diagnosis of an expanding LA mass following CPB, the echocardiographic findings, and intraoperative decision-making are discussed, the likely cause of the injury is identified, and the repair strategy is described.
