Flexible bronchoscopy (FFB) is not necessary for accurate placement of a double-lumen endotracheal tube (DLT). Until several years ago, confirmation of a DLT position was limited to inspection and auscultation. FFB was available in the bronchoscopy suite for the exclusive use by the pulmonary personnel. Today, in most institutions, flexible bronchoscopes of different diameters are available in the operating room for use by the anesthesia personnel. The advances in technology and improved quality of the endoscopic image make the technique easier to use with a relatively simple learning curve. To facilitate the learning process, many workshops are offered by various professional organizations. Therefore, the rationale for the use of FFB for correct DLT positioning has changed in the past 10 years. Before one can judge whether DLTs should be positioned with the flexible bronchoscope, some issues should be considered.
