Reticular crack lesion in the ocular fundus.

An unusual appearance of the ocular fundus in a sporadic case is presented. No identical case was found in the literature reviewed.A 30-year-old Japanese woman complained of blurred vision in her right eye after a recent ocular bruise. Her visual acuity was 0.04 (1.2 x -8.5 D) OD and 0.15 (1.2 x -5.0 D) OS. Anterior segments were normal. Ocular tension was 12 mmHg in both eyes. Fundus examination showed a mild retinal concussion on the right macula. After a few days, the retinal concussion was reduced and the patient recovered her vision. Ocular fundus photographs revealed symmetrical fine, white, crack-like reticular lesions at the level of the retinal pigment epithelium in the posterior pole (Fig. 1). Foveal reflex, optic nerve heads, and retinal vessels were normal in both eyes. Further interview revealed that she had been asymptomatic except for mild photophobia since her twenties. Electroretinographic responses, visual fields, and color vision were normal. As she hoped to become pregnant, fluorescein angiography, which poses a risk of systemic side effects, was rejected. There was no parental consanguinity or remarkable family history of hereditary diseases. She was systemically normal and had never had any ophthalmic surgery.
