Pharmacy Automation and Technology-Pharmacy, Health Information Technology, and the Medical Home: Achieving the Whole Picture

This installment addresses a topic that is expected to grow in importance and relevance to pharmacists, other providers, and patients in the coming months and years. Patient-centered medical homes are being positioned as a significant change to primary care. Health information technology (HIT) is expected to be an important component of patient-centered medical homes, but the presence of HIT does not transition a primary care service into a patient-centered medical home. We discuss the patient-centered medical home concept, the role of HIT, and the role of pharmacists.
