I thank Malcolm Macmillan for his close attention to my article, 'The bones of the insane'. I am aware of Professor Macmillan's own research into the life and work of Alfred Campbell, which I very much look forward to reading in due course, though Campbell himself forms just one part of the story that I discuss in 'The bones of the insane'.
