Early repolarization syndrome (ERS) is a primary electrical disease characterized by early repolarization (ER) in the surface ECG and an increased risk of sudden cardiac death because of VF. Experimental studies have suggested that a prominent transient outward current-mediated action potential notch in epicardial cells creates a transmural voltage gradient that results in ER in ERS. Ghosh et al studied 2 patients with idiopathic VF and ER by using noninvasive electrocardio-graphic imaging and demonstrated that their depolarization was normal (homogeneous) but their repolarization was indeed "early" (premature) in some areas that were adjacent to normal repolarizing zones. However, no clinical study has yet been reported that has directly recorded and examined an epicardial electrogram of the LV in patients with ERS.
