Intraoperative transfusion, triggers and precision--an oxymoron.

Assessing appropriate transfusion in 'cold ' situations should and does now reflect the acceptance of current guidelines. What is far more difficult to assess is appropriate blood usage in dynamic acute circumstances. Using the recommended triggers to define 'appropriate', Desalu and his colleagues report an appropriate transfusion rate of 61-8% intraopera-tively, using clinical assessment methods alone, and question 'how precise are we?' (Desalu et al., 2008).
