Positive HBV-NAT in a blood donor 11years after HBV vaccination

The vaccination for hepatitis B virus (HBV) can prevent clinical hepatitis B, but may not always prevent transmission.Sanquin Blood Supply Foundation provides on a nonprofit basis blood and blood products for patients in the Netherlands. After selection of donors by risk evaluation all donations are screened for HBV, HCV, HIV and Treponema pallidum in the national screening centre. In 2008 HBV-nucleic acid amplification test (NAT) and in 2010 hepatitis B core antibody (anti-Hbc) screening was introduced, in addition to the screening for hepatitis B surface antigen (HBsAg) since 1973. In the case of a positive screening test confirmatory testing is done on the donation sample. When this confirmation is positive the donor is counselled by a donor physician and a new sample for re-confirmation of the positive test result is obtained. The counselling is done using a standard questionnaire to identify risk factors for infection.
