Thrombotic thrombocytopenic purpura following post-ERCP pancreatitis

To Editor, Thrombotic thrombocytopenic purpura (TTP) is a syndrome characterized by fever, microangiopathic hemolytic anemia, thrombocytopenia, renal disease and neurologic changes. TTP constitutes a poorly understood multisys-temic disease of vascular origin that may involve any organ by thrombotic occlusions of the small vessels. It may be idiopathic or secondary. There is a range of clinical conditions that trigger the onset of acute TTP. TTP is an unusual complication (approximately 2.0%) of acute pancreatitis. Acute pancreatitis (AP) is the most common (1-5%) serious complication of Endoscopic Retrograde Cholangio-pancreatography (ERCP). An association of TTP with post-ERCP pancreatitis has previously been reported in three cases. Herein we report two patients who developed TTP presenting with post-ERCP pancreatitis.
