Preservation of the subvalvulai apparatus during mitral valve replacement (MVR) was originally suggested by Lillehei and colleagues in 1964, and the superiority of this approach over MVR with chordal resection has been demonstrated. Several different techniques may be used to preserve the mitral leaflets; however, complete preservation of 1 or both leaflets is associated with valvular thrombosis, which may require early explantation We report 2 cases of an additional complication to such an approach: severe mitral stenosis caused by adhesion of the preserved valvular apparatus to the bioprosthesis#
