Ulnar artery pseudoaneurysm following single non-penetrating trauma to the hypothenar region

Dear Sir, We present a rare case of ulnar artery pseudoaneurysm following a single episode of non-penetrating trauma. A 49-year-old right-hand dominant male who was working as a social housing officer, presented to the hand surgery unit with five-day history of pain and swelling overlying the hypothenar eminence of his left hand. He described an episode where he fell onto his left hand while outstretched ten days previously. Initially he had no ill effects following the fall, but five days later developed a localized area of swelling measuring approximately 2 cm x 2 cm, at the level of the hypothenar eminence in the same hand (Figure 1). His main symptom was of worsening pain overlying the area of swelling. He was a heavy smoker, who was otherwise fit and well, with no other co-morbidities. On examination, the area of swelling was noted to be expansile, and markedly tender on palpation. In addition he had some pain on flexion of his little and ring fingers. A full range of motion at the level of the wrist and all digits was noted. There was no neurovascular compromise, and an Allen's test suggested adequate collateral supply from the radial artery.
