The Journey to Becoming a Medical Home

Nurse practitioners (NPs) provide quality healthcare and are well-qualified to lead a healthcare team. In New Hampshire, NPs have plenary authority, which enables them to establish NP-led private practices in primary care. The pinnacle of primary care is the medical home, and NPs have now proven that they are fully qualified to lead a primary care medical home. This article describes the journey of a small, rural, NP-owned primary care practice on its path to recognition as a medical home.
