Managing risks in drug therapy

In recent years, increasing attention has been focussed on medication errors as a cause of iatrogenic disease. This paper outlines the magnitude of adverse drug events and medication errors and describes the approach adopted by the Copenhagen Hospital Co-operation in order to address these problems. The Copenhagen Hospital Co-operation (H:S for short) is a public health organisation that owns and operates six hospitals serving approximately 20% of the patients in the Danish health care system. H:S is the first hospital system in Scandinavia to be accredited by the Joint Commission on the Accreditation of Healthcare Organisations (JCAHO).
