Paediatric registrars' perceptions of education and training opportunities

The Caiman Reform of Higher Specialist Medical Training in the 1990s brought British specialist training in line with European medical directives.1 The two previous higher specialist training grades (registrar and senior registrar) were combined into a unified specialist registrar grade. The curriculum and minimum training period for each specialty was defined, and educational supervision was strengthened with induction for all posts, supervised learning and regular reviews of progress. Ongoing implementation of the European Working Time Directive (EWTD) saw the number of working hours being reduced stepwise to a maximum of 56 hours per week by August 2004, to be reduced further to 48 hours per week in August 2009. Concerns have been expressed that the reduction in specialist registrar (SpR) working hours is having a detrimental effect on training opportunities, patient care and doctors' lives outside their work.
