Comment on: Open intraperitoneal versus retromuscular mesh repair for umbilical hernias less than 3 cm in diameter

In the January issue of The American Journal of Surgery, Berrevoet et al report on 116 patients who had umbilical hernia repair. They compare a retromuscular repair using Vypro mesh (Ethicon, Livingston, Scotland) with an intra-peritoneal repair using the Ventralex mesh (Bard, Helsing-borg, Sweden). The Vypro repair took over twice as long, and the hospital stay was longer than the Ventralex group. There were more seromas in the retromuscular Vypro group. Postoperative pain was significantly lower in the intraperitoneal Ventralex group. There were 5 recurrences in the Ventralex group and 2 in the Vypro group, which was not statistically significant.
